Community First Services and Supports (CFSS) is Minnesota’s new self-directed home care program, designed to help individuals with disabilities, chronic illnesses, or age-related limitations live more independently at home and in the community.
CFSS is replacing the Personal Care Assistance (PCA) program and offers greater flexibility, more choice, and increased control over your care. Whether you need help with daily activities like dressing, bathing, meal prep, or managing health-related tasks, CFSS is built to support your unique needs.

What Does CFSS Include?
- Help with activities of daily living (ADLs) such as bathing, dressing, eating, mobility, and toileting
- Support with instrumental activities of daily living (IADLs) like meal prep, laundry, and light housekeeping
- Health-related tasks (under nurse delegation)
- Personal assistance customized to your lifestyle
- The ability to choose and train your caregiver (including family members, with some exceptions)
