Individual Community Living Supports (ICLS) is a home and community-based service that provides support to older adults and individuals with disabilities, helping them maintain their independence and continue living in their own homes.
ICLS is focused on teaching, supporting, and empowering individuals to manage their daily lives while building skills and confidence over time.

Who Is Eligible?
You may qualify for ICLS if you:
- Are on Elderly Waiver (EW) or Alternative Care (AC) Program
- Live in your own home or apartment (not assisted living)

- Are referred through your case manager or care coordinator
